What The Large Cohort Study Actually Found

Researchers pulled records from the TriNetX database, covering men age 40 and older treated for lower urinary tract symptoms between 2012 and 2022. They matched 36,927 tadalafil users against an equal number of similar men who did not take the drug. Over five years, tadalafil users showed a 22% higher relative risk of developing glaucoma, according to the published findings.

The danger wasn’t limited to one type of glaucoma. Ocular hypertension risk rose 32%, and primary open-angle glaucoma, the most common form of the disease, rose 33% among tadalafil users. Doctors also noted these men started glaucoma treatments more often than the comparison group, a sign their eye pressure problems were real enough to need medical attention.

Why Doctors Aren’t Ready To Blame The Drug Outright

Here’s the catch that matters most: this was an observational study, not a controlled experiment. Researchers watched what happened to real patients over time, but they didn’t randomly assign some men to take tadalafil and others a placebo. Because of that, a doctor quoted in coverage of the findings said the study “couldn’t draw a direct cause-and-effect link between Cialis and glaucoma”.

The study’s own authors admitted their design has limits. Review of Optometry reported that the researchers acknowledged their retrospective, observational approach “prevented their ability to draw causal conclusions”. They even suggested the pattern they saw was “unlikely to be mediated through direct effects on aqueous humor dynamics,” meaning the drug may not be squeezing the eye’s fluid system the way glaucoma usually develops.

What This Means For Men Weighing The Tradeoff

This isn’t a reason to panic, but it is a reason to talk to your eye doctor if you’re on long-term tadalafil, especially if glaucoma runs in your family or you already have elevated eye pressure. The study’s authors themselves recommended ophthalmic evaluation and follow-up for higher-risk patients. That’s a reasonable, low-cost precaution while science keeps working out whether this signal is a real threat or a statistical mirage.

What’s clear is that men deserve honest information, not hype in either direction. A 22% relative increase sounds alarming in a headline, but glaucoma remains uncommon enough that the actual number of extra cases stays small. Patients and doctors should weigh real benefits against a possible, unproven risk, and regular eye exams are cheap insurance against the worst outcome, permanent vision loss.
