What The New Study Actually Found

Researchers led by Maggie Fuzak Nunziato at George Mason University tracked data from the Utah Population Database between 1996 and 2021. They compared health outcomes for women with and without endometriosis over a quarter century. The result: women with endometriosis carried a 46% higher risk of developing type 2 diabetes after adjusting for other factors.

The study, published in the journal Diabetologia, is described by George Mason as the largest of its kind to examine this link. A companion analysis, part of the same research effort, reported an adjusted hazard ratio of 1.46, meaning women with endometriosis were 46% more likely to develop diabetes than women without it, with a tight confidence range supporting the finding.

Not Every Patient Faces The Same Risk

The numbers were not uniform across every woman studied. Researchers found the strongest diabetes risk showed up in what they call “other site” endometriosis, meaning tissue growth outside the typical pelvic locations. That pattern suggests the disease itself behaves differently depending on where it takes hold in the body, not as one single uniform condition.

Premenopausal women also showed more pronounced associations than women past menopause. That detail matters because it points researchers toward hormonal activity, not just age or accumulated years of disease, as a driver behind the elevated diabetes numbers. Doctors treating younger patients with endometriosis now have a clearer reason to watch blood sugar closely.

How This Fits With Earlier Research

This is not the first time scientists have looked for a connection between endometriosis and metabolic disease, and earlier studies told a messier story. The Nurses’ Health Study II, which followed more than 112,000 women, found no significant overall link between laparoscopically confirmed endometriosis and type 2 diabetes once researchers adjusted for other health factors.

A separate French cohort study, known as E3N, tracked women averaging 51 years old and also found no significant association after adjusting for body weight, physical activity, smoking, and other lifestyle factors. Those earlier results leaned null, while this new, much larger study leans strongly positive, showing how much scale and population differences can shift a result.

Why The Metabolic Connection Makes Sense

Endometriosis has already been tied to broader metabolic trouble beyond diabetes alone. One analysis found women with the condition faced higher odds of metabolic syndrome, a cluster of issues including high blood pressure, excess body fat, and abnormal cholesterol that often precedes diabetes. That overlap gives doctors a reason to treat endometriosis as more than a reproductive issue.

The George Mason team was careful to frame their finding as an association, not proof that endometriosis directly causes diabetes. Biology is complicated, and chronic inflammation, hormone imbalances, and shared risk factors could all play a role. Still, a 46% higher risk in a study of nearly 3 million women is not a number doctors or patients should brush aside.

What This Means For Women And Their Doctors

An estimated 10% of reproductive-age women worldwide live with endometriosis, a chronic condition already linked to pain, fertility struggles, and now a heightened diabetes risk. This study gives physicians a stronger case for screening these patients earlier and more often for blood sugar problems. For millions of women already managing one difficult diagnosis, catching a second one early could make all the difference.
